Log a Vacation Reminder

Reminder... This is NOT a request form, It is a reminder to the rest of the team in your Hub, so that a Priority One job is NOT scheduled while you are away.  Note, scheduling complications occasionally will occur... In that instance, coverage for you on the conflicting Priority One job will need to be logged.
